The Virginia Department of Health (VDH) has opened the Virginia Rural Health Transformation Program (RHTP) Community Paramedicine & Mobile Integrated Healthcare Request for Applications, reference VDH-26-517-0386. The RFA was issued September 4, 2026 and supports community-driven CP/MIH models serving federally designated rural Virginia communities.
Funding and Deadline
VDH anticipates approximately $3.7 million in competitive subawards during the initial performance period. Applicants may request up to $400,000 per application. Applications are due October 6, 2026 at 5:00 PM ET through the eVA procurement portal. Questions are due September 25, 2026 at 5:00 PM ET, and VDH will post responses in an addendum after the Q&A period. VDH held the pre-application webinar on September 16, 2026 at 1:00 PM ET; applicants should use the active Q&A period through September 25, 2026 at 5:00 PM ET and review the resulting eVA addendum when posted.
Eligible Applicants
Applicants must be legally organized entities in good standing capable of receiving and administering a federal subaward. Named eligible types include licensed EMS agencies, nonprofits, state/local/Tribal governmental entities, hospitals and health systems, FQHCs, Rural Health Clinics, institutions of higher education, and other organizations with demonstrated capacity to implement CP/MIH programs serving rural communities. Applicants must maintain, or be in the process of obtaining, SAM.gov registration with a current UEI before final award, demonstrate 2 CFR Part 200 grant-management capacity, directly serve one or more FORHP-designated rural Virginia localities, and demonstrate the clinical and operational capacity and partnerships needed for the proposed model.
No cost sharing or matching funds are required. The initial subaward period runs from execution through September 30, 2027, with continuation potentially available through September 30, 2030 subject to funding and performance.
Technology, Equipment and N9+ Fit
The controlling RFA expressly supports telehealth, remote patient monitoring, digital health technologies, community-based assessments, chronic disease management, post-discharge follow-up, care transitions, medication management, preventive services, behavioral health support, maternal and child health services, and other CP/MIH interventions. The budget instructions expressly contemplate contractual/consultant services and equipment purchases when reasonable, necessary, allowable, and directly related to implementation.
Atlas Opportunity Score: 99/100. N9+ Fit Score: 97/100 — HIGH FIT. The N9+ is a strong, legitimate fit where an eligible rural Virginia lead applicant uses connected physical examinations, vitals and clinician-guided remote assessment to strengthen treat-in-place, community paramedicine, mobile integrated healthcare, telehealth, chronic-care, post-discharge or rural-access workflows. Dr. Miltie’s strongest current capture path is as a technology vendor or implementation partner under an eligible CP/MIH lead. A direct Dr. Miltie subrecipient application is conditional, not yet TRUE, because the applicant itself would need to demonstrate CP/MIH organizational, clinical and operational capacity, rural Virginia service capacity, federal subaward readiness and required partnerships. The RFA does not require match, so match is not a direct-applicant blocker.
Geographic and Competition Notes
The competition is open to eligible applicants across rural Virginia, but initial awards will be concentrated in three Virginia Regional EMS Council service areas selected through competitive review. Funded activities must directly benefit FORHP-designated rural Virginia communities listed in the RFA appendix.
Application Operations
VDH identifies Brandon Rivenbark, Rural Hospital Program Manager, as the program contact. Applicants should use the controlling RFA, all eVA postings and future addenda when preparing a submission. eVA has a scheduled blackout from September 10, 2026 at 7:00 PM ET through September 14, 2026 at 7:00 AM ET; VDH states that the RFA documents will remain available through its Rural Health Transformation resource page during that period.
