Qualification and Activation Requirements
The controlling Open Enrollment requires at least five years of relevant experience. Each participating pharmacy must maintain required permits, licenses and certifications in current good standing. DSHS may activate services at any time, including nights, weekends and holidays, and the contractor must acknowledge activation within one hour. At activation, the contractor must provide each participating pharmacy’s location, address, email, contact and 24/7 phone information and make reasonable efforts to coordinate prescription delivery.
Pre-award review includes Texas debarment, federal SAM exclusions, applicable statutory divestment checks, Texas HHS OIG and U.S. HHS OIG exclusion screening, Texas franchise-tax and Secretary of State standing, warrant-hold review and vendor-performance history. Unfavorable VPTS history can support denial. Required application materials also include experience documentation, licensure policy/procedure, organizational chart and key-staff resumes, executive summary/approach, applicable insurance and the official HHS exhibits and forms.
