The Wisconsin Department of Health Services (DHS) issued its Asthma Education Services Provider Capacity Building Request for Applications on October 19, 2026. This is an applicant-facing, competitive grant under the Wisconsin Asthma-Safe Homes Program, supported through the Medicaid/CHIP Children’s Health Insurance Program Health Services Initiative. Its purpose is to help eligible Wisconsin community organizations prepare to deliver in-home asthma education and trigger assessments, including a small pilot serving eligible households. This is not a commercial procurement contract and is not a Rural Health Transformation Program (RHTP) solicitation.
Funding and award size
DHS expects to provide up to $80,000 in combined program funding through up to two grants of approximately $40,000 each, subject to funding and final negotiations. $80,000 is the overall pool, not an individual applicant award. The funds support the period January 1 through September 30, 2027. Awards are reimbursement-only for actual eligible costs incurred; applicants must have sufficient working capital to pay staff and expenses before invoicing. No matching funds are required, and the award does not carry forward after the grant term.
Who can apply
Eligible applicants must be located in Wisconsin and provide community-based services in Brown County or Milwaukee County. They must be a qualifying 501(c)(3) community-based organization, local or tribal health department, community-based healthcare organization, or Medicaid managed care organization, with technical and administrative capacity to complete the work, culturally and linguistically appropriate services, and an intention to apply to become an Asthma-Safe Homes Program education services provider during the next grant period. The population served includes children ages 2–18 and pregnant people with asthma who are enrolled in BadgerCare Plus.
Fundable activities and restrictions
Projects must build organizational capacity and pilot in-home asthma education for up to ten clients. Activities include staffing and workflow development, identifying referral partners, outreach and marketing plans, training and grantee meetings, two to six education visits per client, follow-up calls, and home asthma-trigger assessment. Approved reimbursable costs include eligible project staff and fringe, mileage, supplies and materials, training stipends up to $500 per staff member, and eligible client services, including asthma-trigger reduction supplies up to $1,000 per household. DHS lists 10% indirect administrative costs as an eligible budget category, subject to the approved award. Work outside the grant period and expenses not approved under the award should not be assumed reimbursable. There is no advance payment, carryover funding, or unrestricted equipment-purchase entitlement.
Deadline, application package, and selection
Applications must be submitted online by November 13, 2026, at 5:00 p.m. Central Standard Time (CST). The application must include organization/contact details, scored narrative responses addressing experience, readiness and capacity building, partnerships, and community engagement, along with the organization’s most recent financial statement and resumes of key personnel. DHS anticipates notifications during the week of November 30, 2026. Applications are scored out of 50 points and DHS may negotiate final award amounts. Applicants should review all current official instructions and updates before submitting.
