MDH lists $561,000 in total funding, expects 10 to 13 awards, and sets a $45,000 maximum award. Minnesota Department of Health’s FY2027 Community Clinic Grant supports eligible clinics serving low-income and rural populations, with FY2027 funding focused on increasing or maintaining access to health services for uninsured and underinsured patients.

The current MDH program page verifies the eligible clinic classes, FY2027 funding level, award maximum, deadline, portal submission path, listed application materials, a current CCG Questions and Answers document, and an informational webinar scheduled for September 24, 2026 at 11:00 AM CT. A potential applicant-side path for the Nonagon N9+ remote physical examination platform may exist where an eligible clinic can tie remote clinical capability directly to increased or maintained access for uninsured or underinsured patients, and the current FY2027 RFP confirms no match is required and expressly allows equipment, instruments, supplies, EHR or billing-system updates, translation and interpretation, culturally specific outreach, staff time, and other eligible access-support costs when tied to the funded project. Dr. Miltie is not an eligible direct grant applicant.

MDH will hold an informational webinar on September 24, 2026 at 11:00 a.m. Central Time. Applications must be submitted through the ORHPC Online Grants Portal by October 30, 2026 at 4:30 PM Central Time.
