Broad Eligibility

For Budget Period 1, eligible lead applicant types are Virginia hospitals and health systems that are licensed or authorized to provide healthcare services in Virginia and serve qualifying rural Virginia communities. Atlas Analysis

Atlas Opportunity Score 99/100. N9+ Direct Vendor Fit 88/100, HIGH FIT under a partner required vendor model. The eligible lead must be a Virginia hospital or health system serving qualifying rural populations; Dr. Miltie is not the lead applicant. The RFA expressly encourages RPM vendor partnerships, requires applicants to identify projected RPM vendors and their procurement approach, and requires purchase and deployment of RPM technologies and devices. Position the Nonagon N9+ device and platform only as a potential complementary remote physical examination component within a broader eligible RPM and care management model, subject to applicant clinical design, interoperability, privacy and security review, procurement, VHHAF allowability, and approved budget. Do not characterize N9+ as continuous or passive monitoring. Preserve non supplanting, third party payer, rural geography, procurement, and cost reimbursement controls.

The Virginia Hospital & Healthcare Association Foundation issued RFA-RHTP-RPM-1 for Virginia Rural Health Transformation’s CareIQ Remote Patient Monitoring initiative. The official RFA makes $14.3 million available among awardees for Budget Period 1, estimates two to three awards, and sets a September 15, 2026 at 11:59 PM Eastern application deadline.

The RFA focuses on implementing or expanding hospital-based RPM for rural patients, particularly people with chronic disease or recently discharged patients requiring close follow-up. Required activities include RPM technology and device deployment, clinical-system integration, monitoring workflows, staff training, patient enrollment, outcomes reporting, provider coordination and a sustainability plan. Applicants should use RFA-RHTP-RPM-1 and any subsequent VHHAF or DMAS amendments as the controlling submission source.
