Broad Eligibility
Applicants that timely submitted the mandatory August 10 Letter of Intent may pursue a rural Iowa co-location site with at least three service-delivery entities in one location. Eligible partner types include FQHCs, CHCs, RHCs, hospitals, physician groups, behavioral health, substance-use treatment, pharmacies, oral health, maternal and child health, community organizations, rehabilitation, urgent care, laboratory and diagnostic providers, and other qualifying services.

Strong integrated-care opportunity for organizations that already cleared the LOI gate and can assemble a credible multi-provider rural site. The highest-value projects should demonstrate true operational integration, signed partner commitment, chronic-disease impact, complementary provider roles and a sustainable shared-service model rather than simple proximity.
Iowa HHS issued RFP COMPADM26005 to establish pilot rural Communities of Care Co-Location Sites. Each proposed site must bring together at least three service-delivery entities at a single rural location, with at least one entity providing chronic-disease prevention or management services.
The mandatory Letter of Intent was due August 10, 2026 at 4:00 PM. The September 1 application window therefore remains actionable only for applicants that satisfied that gate. Signed MOUs with proposed service-delivery partners are required with the application.
