Broad Eligibility

Qualified cardiovascular screening and/or follow-up providers. Category A clinics must be physically located in Daggett, Duchesne, Summit, Tooele, Uintah, Utah, or Wasatch counties and use Application/SOW A. Category B clinics must be physically located in Salt Lake County and use Application/SOW B. Applicants must satisfy all applicable qualification requirements.

Atlas Analysis

Qualified clinics may apply year-round to join Utah DHHS BeWise as cardiovascular screening and follow-up providers. The current application page remains open year around and provides separate applications and scopes, a submission checklist, conflict-of-interest disclosure, W-9, approved CPT codes, and program policies. Treat this as a provider-contract opportunity: verify clinic geography and category, service qualifications, reporting and billing obligations, and currently approved services before applying.

Opportunity Overview

The Utah Department of Health and Human Services is accepting applications year-round for DHHS26081 Cardiovascular Screening and Follow-up Services through Utah’s BeWise program. The open-ended Invitation for Applications seeks qualified providers to deliver approved cardiovascular screening and/or follow-up services for eligible Utah participants. DHHS evaluates applications as received and anticipates awarding evergreen contracts with no end date to all qualified providers.

Funding and Deadline

This opportunity has no set closing date. DHHS does not publish a total program ceiling or universal contract maximum. Contracts use pre-established DHHS reimbursement rates, and applicants must not submit their own pricing. Published allowable CPT rates include office visits up to $120 and separate laboratory/service rates. A contract award does not guarantee service volume.

Regional Eligibility

Category A applies to clinics physically located in Daggett, Duchesne, Summit, Tooele, Uintah, Utah, or Wasatch counties. These applicants use Application A and Scope of Work A. Every qualification question must be answered yes, including service to low-income, uninsured, or underinsured patients; qualifying Utah-licensed MD/DO/APRN/PA providers; evidence-based cardiovascular care; team-based care; capacity to schedule alert-value follow-up within seven calendar days; EHR and secure electronic record/billing capability; Medicare-rate acceptance; and the required clinical reporting and billing commitments.

Category B applies to clinics physically located in Salt Lake County. These applicants use Application B and Scope of Work B. Category B additionally requires the clinic to be an HRSA Federally Qualified Health Center and currently provide breast and cervical cancer screening through Utah DHHS. Applicants must satisfy every qualification in the applicable application; Atlas does not presume eligibility from geography alone.

Clinical Scope

The BeWise program is designed to improve cardiovascular health for eligible mid-to-low-income women ages 35-64. The scopes include provider-led risk-reduction counseling, follow-up services, chronic-disease management for abnormal blood pressure, cholesterol, glucose, or A1C values, reporting, team-based-care processes, and Automatic Office Blood Pressure implementation. Category B also includes cardiovascular screening measurements such as height, weight, blood pressure, hip and waist measurements, lipid testing, and fasting glucose or A1C.

N9+ Fit Score: 70/100, conditional. The Nonagon N9+ remote physical examination platform can plausibly support guided remote examination, vitals, remote assessment, and telehealth-enabled follow-up where those workflows are clinically appropriate. It cannot replace required laboratory testing, clinic qualifications, program-manual requirements, or other BeWise compliance obligations. Technology allowability should be confirmed before budgeting or representing N9+ as part of a BeWise project.

Submission Requirements

Applications must be submitted by email to the BeWise address identified in the official IFA. The subject line must include the provider business name and DHHS26081 BeWise IFA. The submission checklist requires the Conflict of Interest Disclosure Statement, W-9, and the correct Application Form A or B for the clinic’s location. Applications submitted by another method or to another email address are not considered.

Atlas Applicant Strategy

Atlas Opportunity Score: 88/100. This is a strong evergreen opportunity for qualified Utah clinics that already serve the target population and can meet the clinical, reporting, billing, team-based-care, and regional requirements. Atlas has identified Mountainlands Community Health Center as a high-potential Category A candidate based on its Utah County presence and verified primary-care capabilities, but every mandatory qualification must be confirmed before any application or outreach is advanced.
