Current Rural Health Transformation Grant Opportunity
The Accomack-Northampton Planning District Commission (A-NPDC) and Eastern Shore Regional Planning District Collaborative are soliciting competitive applicant-facing subaward proposals under Virginia Rural Health Transformation Program (RHTP) Mobile and Hybrid Care, reference RFA-RHTP-2026-01 (issued October 23, 2026). The purpose is to improve access to primary, preventive, behavioral health, substance-use, dental, vision, specialty, screening and diagnostic care for eligible rural Virginia residents through sustainable mobile and hybrid delivery models. This is competitive financial assistance to qualifying applicants, not an equipment-procurement or consulting contract.
Funding Amounts, Performance Period and Deadline
The official RFA identifies approximately $11,800,000 in total available program funding and anticipates at least five competitive awards. There is no established maximum award per individual application. The $11.8 million figure is an aggregate competition pool and must not be construed as the available amount for one applicant. Awards may be partial or negotiated. Applicants must propose a reasonable, necessary, documented first-year project budget; all year-one award costs must be incurred within the authorized contract period and spent by September 30, 2027. Funding is reimbursement only, with no advances; applicants need sufficient cash flow. The RFA does not prescribe a general mandatory matching percentage, but other funding sources should be disclosed.
Application deadline: Monday, November 23, 2026, at 5:00 p.m. Eastern Time. Written questions must reach russwilliams@esvaplan.org by Friday, November 20, 2026, at 5:00 p.m. Eastern Time. Anticipated award notifications are January 8, 2027. Applicants should check the administrator website for posted addenda and question responses.
Eligible Applicants and Eligible Rural Service Area
Qualifying lead applicants must be legally organized, in good standing, capable of administering a federal subaward under 2 CFR Part 200 and able to document financial, administrative, reporting and reimbursement cash-flow capacity. Potential applicant types include nonprofits, government entities, health systems, Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), educational institutions, healthcare providers, community-based organizations, mobile health technology organizations and other capable entities. Applicants may apply alone or with partners. Direct clinical care must be provided by properly licensed and credentialed organizations or documented clinical partners.
Projects must directly serve eligible rural residents within one or more of the following Virginia jurisdictions, subject to the applicable federal rural eligibility definition: Lancaster, Northumberland, Richmond, Westmoreland, Essex, King and Queen, King William, Mathews, Middlesex, Accomack, Northampton, Southampton and Surry Counties, and the City of Franklin. Applicant headquarters need not be located within the region if the proposed project directly benefits its qualifying rural residents. Serving a locality by itself does not excuse the need to verify rural-population eligibility.
Allowable Activities and Material Restrictions
Potentially allowable startup and implementation expenses include mobile care vehicles and equipment, diagnostic equipment, telehealth kiosks, clinics in a box, hub-and-spoke care infrastructure, electronic consultations, time-limited initial personnel, data systems, clinical training, technical assistance and necessary minor renovations consistent with award rules. Proposals must document needs, implementation milestones, measurable rural-resident impact, partner roles, sustainability and compliance.
Prohibited costs include pre-award expenses, new construction, supplanting existing funding, costs otherwise paid by Medicare, Medicaid or another payer, lobbying, profits, and expenses unrelated to the approved project. Indirect costs are not available; direct administration funding is expected to be minimal. Capital improvements that materially extend asset value or useful life require applicable prior approval. Awardees must comply with financial and program reporting and federal subaward requirements.
How to Apply
Prepare the completed Appendix A application cover sheet, a project narrative of no more than 15 pages, implementation work plan and performance measures, detailed project budget and narrative, the most recent audit or financial statements as applicable, clinical licenses where relevant, material partner commitment letters, and required certifications/disclosures. A Unique Entity Identifier (UEI) is required if an award is made but the RFA explicitly says UEI documentation is not required at the application stage. Applicants should nevertheless confirm any later posted clarifications and prepare UEI registration early. Submit one complete PDF by email to russwilliams@esvaplan.org before the published deadline and obtain confirmation of successful receipt.
