Required next actionConfirm the project fits the 2026 CMP quality-of-care or quality-of-life focus and allowable-cost rules. If the applicant is not a nursing home, secure signed letters of support from every participating nursing home with the required CCN, project timeframe, budget-cap acknowledgment and commitment. Complete the CMS CMP application, competitive requirements and grant budget, then submit through Tennessee's online CMP submission form by September 25, 2026 at 2:00 PM CT.

Broad Eligibility

Eligible applicants include licensed and Medicaid-certified Tennessee nursing facilities, healthcare systems, state agencies, for-profit and nonprofit organizations, provider associations and universities. Projects must benefit residents of Tennessee nursing homes and satisfy the current CMP Reinvestment Program requirements. Non-nursing-home applicants must provide required support letters from every participating nursing home.
