Eligible applicants may include healthcare providers and health systems, primary care practices, Federally Qualified Health Centers, behavioral health and substance-use organizations, dental providers, EMS organizations proposing activities that do not establish or duplicate a separate MIH/community-paramedicine program, public agencies, community-based organizations, nonprofits, public-private partnerships and qualifying for-profit entities. Applicants should demonstrate service to St. Mary's County, Southern Maryland, rural or underserved populations; for-profit budgets may not include profit, fees or unallowable margin.

St. Mary’s County Health Department issued RFP 27-05 to fund mobile, home-based, community-based and place-based health access strategies for county residents under Maryland’s Rural Health Transformation Program. The controlling RFP anticipates up to approximately $700,000 across one or more awards and describes digital-health, telehealth, remote patient monitoring, community micro-access and coordinated-navigation models as eligible approaches.
Projects must function as additive strategy arms of St. Mary’s 360, use the county Health & Wellness Portal when applicable, support closed-loop referral and follow-up, and cooperate with SMCHD, MDH, CRISP and other designated partners on interoperability, outcome measurement and reporting. The solicitation expressly warns against duplicating a separate Mobile Integrated Health/community-paramedicine program, a separate countywide care-coordination system or a competing community-referral platform.
The project period is anticipated to run October 1, 2026 through June 30, 2027, with four optional one-year renewals subject to funding. The controlling RFP states a September 17, 2026 at 3:00 PM Eastern submission deadline, while the August 27 public announcement states 5:00 PM; Atlas therefore treats 3:00 PM as the safer controlling deadline unless SMCHD publishes an amendment.
