Eligibility

Eligible applicants are 501(c)(3) nonprofit organizations and government agencies that are located in and serve communities or populations within the Foundation’s service area. Applicants must align with the Foundation’s mission and demonstrate strong board governance and financial management. Municipalities within the service area are encouraged to apply.

Churches, faith-based organizations and schools serving the service area may also apply, but their proposals must include a signed joint-use agreement with the applicable city or town for public use of the healthy space and an agreement to post hours during which the space will be available for public use.

Fundable projects and costs

The grant is intended for shovel-ready infrastructure projects such as walking trails, playgrounds, parks, farmers markets, community gardens, healthy corner stores and mobile markets. Grant funds may support capital expenses, equipment costs, and a limited amount of consulting or personnel support and fringe when necessary to implement the project.

Matching funds or in-kind support are encouraged but not required.

Funds may not be used for administrative or overhead fees; new or existing programs, educational activities, conferences, meetings or promotional campaigns; lobbying; biomedical, clinical or educational research; scholarships; endowments; annual fund drives; projects outside the service area; individual assistance; or direct medical or social services already funded through third-party reimbursement.

Submission requirements

Applications are accepted only through the Foundation’s online application process. Late or incomplete proposals are not considered for the applicable cycle. Applicants are strongly encouraged to consult Foundation staff before submitting and, when seeking technical assistance, to engage at least two weeks before the deadline.

The required proposal materials include the official Proposal Cover Sheet; a proposal narrative of up to three pages; a project work plan of up to two pages; the line-item Project Budget Worksheet and brief budget narrative; the applicant’s 501(c)(3) or government entity exemption determination letter; the organization’s most current Form 990 or annual audit/financial report; an online list of board members with names and addresses; and up to three letters of support. Awardees will later be required to provide a signed contract, board resolution, communication guide agreement, and other requested documents or contingencies.
