Broad Eligibility
The lead organization must be an eligible New York safety-net hospital under Public Health Law Section 2825-i and must jointly apply with at least one identified partner organization. Eligible partners may include health systems, hospitals, health plans, residential health care facilities, physician groups, community-based organizations and other healthcare entities. The Department selectively invites Letter of Interest parties to complete a full application after reviewing eligibility and the merits of the proposed transformation.
