Opportunity Overview
New York State Department of Health accepts Hospital Vital Access Provider Assurance Program (VAPAP) applications on a rolling basis throughout the year. VAPAP is a discretionary State-only financial-assistance program for hospitals and health systems experiencing serious financial instability and needing extraordinary support to maintain operations and preserve vital services while pursuing longer-term solutions.
Who Can Apply
Eligible facilities include public hospitals, federally designated Critical Access Hospitals, federally designated Sole Community Hospitals, and qualifying safety-net hospitals that meet New York’s Medicaid, uninsured, dual-eligible or service-area criteria. Eligibility does not guarantee an award because funding decisions remain discretionary.
Financial Distress Screen
As a general rule, NYSDOH expects applicants to demonstrate serious financial instability using recent audited and interim financial statements. The Department identifies key indicators including fewer than 15 days cash and equivalents, a negative operating margin within the last two years, no nonessential assets available to monetize, and no other resources sufficient to meet the facility’s financial need.
How Awards Are Evaluated
NYSDOH evaluates the risk of losing vital services, quality of care, community needs, hospital and regional utilization and capacity, access to critical services such as inpatient behavioral health, substance-use, trauma and perinatal care, availability and growth of ambulatory care, historical and projected financial sustainability, the amount and duration of assistance requested, and the availability of State funding.
Eligible and Ineligible Uses
VAPAP supports costs directly related to sustaining operations and preserving vital services. New York law excludes capital expenditures, including construction, renovation and acquisition of capital equipment or major medical equipment, as well as bankruptcy-related costs. Hospitals should therefore treat VAPAP primarily as an operating-liquidity and sustainability vehicle rather than a capital equipment grant.
Application Readiness
Hospitals should request the current application template from NYSDOH using the VAPAP application-request instructions on the official program page. Required supplemental materials include an organizational chart showing corporate relationships, audited financial statements for the two most recent calendar years, current interim financial statements, and the current operating budget for the facility, parent organizations and affiliated partners. Applicants should also prepare a credible sustainability or transformation plan and clearly identify the intended use of funds.
Atlas Analysis
Atlas Opportunity Score: 94/100. This is one of New York’s most important hospital financial-stability pathways because it can provide short-term liquidity where loss of core services is a genuine risk. Strong candidates should quantify the community consequence of service loss, document their financial position cleanly, and connect requested support to a realistic path toward sustainable operations.
N9+ Direct Vendor Fit: 0/100 for direct equipment purchase. VAPAP expressly excludes capital expenditures and major medical equipment. Any Dr. Miltie involvement would need to arise outside the prohibited capital-cost pathway and be clearly tied to an allowable operating or transformation expense confirmed by NYSDOH; Atlas should not present VAPAP as a device-purchase opportunity.
