The Mississippi Division of Medicaid is currently inviting eligible maternal-health providers and organizations to participate in the Mississippi MOMentum Initiative as Accountable Entities under the CMS Transforming Maternal Health (TMaH) Model.
Funding and model: Mississippi’s TMaH cooperative agreement is $17 million over 10 years (2025-2034). Mississippi states that the model is designed to provide maternal-health providers with upfront funds to invest in prevention, strengthen continuous care for high-risk patients across physical and behavioral health, and reward improved outcomes and cost savings. CMS’s payment design includes Provider Infrastructure Payments for care-delivery transformation and data infrastructure in Model Year 3, followed by upside-only quality and performance incentive payments in Model Year 4.
Current participation window: Provider interest and Accountable Entity attestation are open now. The current Mississippi TMaH page and August 19, 2026 participation notice do not publish a fixed closing date, clock time, or time zone. The current attestation is non-binding and does not itself create a contract, legally binding obligation, or guarantee participation, reimbursement, selection, or funding.
Potential Accountable Entities: Mississippi identifies OB/GYN practices, hospital-based practices, birth centers, family medicine practices, Federally Qualified Health Centers, and Rural Health Clinics that provide the full continuum of perinatal care or partner with other providers to do so.
Portal eligibility gates: The TMaH Attestation link is displayed only when the provider’s Account Administrator is logged into the secure MESA Provider Portal, the provider is associated with an eligible taxonomy, the provider has an active MSCAN contract status, and the provider is not already enrolled in TMaH. Delegate accounts cannot submit the attestation.
Submission: Eligible providers use the MESA Provider Portal, select TMaH Attestation, review the participation statement and terms, check the required attestation box, and save. Mississippi also posts General Interest and TMaH Participation Interest forms on its TMaH program page.

TMaH has a legitimate care-transformation and infrastructure pathway, and CMS specifically contemplates provider infrastructure, data capacity, whole-person maternal care, and telehealth-enabled maternal management. However, Mississippi has not published a provider-specific award cap or a current technology/equipment allowability schedule, and the present attestation is only an expression of intent. Do not assume N9+ or other technology costs are allowable without Mississippi/CMS confirmation.
