The County of Maui Department of Agriculture (MDOA) is accepting applications for its FY2027 Food Is Medicine Grant, supporting healthcare programs that integrate locally grown food and nutrition into plans to prevent, manage, and treat illness across Maui County.
Funding and deadline
MDOA will make multiple awards of up to $100,000 for a 12-month grant period. Applications opened September 15, 2026 and close October 15, 2026 at 4:30 p.m. Hawaiʻi Standard Time.
The program uses a cost-reimbursement structure. The controlling materials do not state a fixed mandatory match or cost-share percentage. Applicants are, however, asked to explain self-sufficiency and other funding sources that may leverage County funds.
Who can apply
MDOA will only award eligible applicants operating in Maui County. The Grant Guide states that the applicant must be a health professional with experience working with patients to support improved health outcomes. Eligible legal structures include Hawaiʻi for-profit organizations, IRS-exempt nonprofit organizations, and sole proprietors. The application requires a medical license and other compliance documentation.
What the grant supports
Supported Food Is Medicine models include locally grown fruit and vegetable prescription boxes, local protein boxes, vouchers for qualifying local foods, and medically tailored meals prescribed or guided by a doctor or nutrition professional. Grant-funded food must meet the program’s Maui County sourcing requirements.
The Grant Guide allows eligible project costs including participant food, personnel, certain contractors and consultants, and qualifying equipment. Indirect costs may be charged up to 15% of the total budget. Equipment must be necessary to the project, dedicated solely to the grant, justified, and approved by MDOA.
Application and submission
Applicants must use the County’s official application forms and budget workbook. The Guide permits submission by email to the MDOA Agriculture Grants Team, hand delivery, or mail, subject to the October 15 deadline and the solicitation’s timing requirements. Required supporting materials include compliance/tax documentation, business registration or good-standing documentation, W-9, bylaws or operating procedures, applicable corporate authorization, two years of financial statements, a medical license, and applicable subcontractor or fiscal-sponsorship agreements.
