The Health Trust is accepting project concepts on an ongoing basis for its Health Partnership Grants, which support medically related projects that advance health equity in Santa Clara County and Northern San Benito County. The program prioritizes Policy, Systems, Practice Change and Programmatic Support projects aligned with Food and Nutrition, Chronic Disease Prevention and Management, or Supportive Housing. General operating support may occasionally be considered when an organization’s core work aligns with a focus area.
Eligible applicants are public agencies and nonprofit organizations that are tax exempt at both the federal and California state levels. Projects must directly benefit residents of Santa Clara County and/or Northern San Benito County. Priority is given to organizations addressing documented health disparities, communities experiencing deep inequities, and applicants that demonstrate the capacity and community relationships needed to execute the project. Proposals must include SMART outcomes.
The Health Trust does not publish a maximum applicant request for Health Partnership Grants on its current application page. It reports investing more than $2 million annually in nonprofit and public agencies, and its fiscal year 2025 grantmaking included 25 Health Partnership Grants totaling $3.55 million. Those historical totals are context only and are not an applicant award guarantee or published cap.
Concept Forms are accepted and reviewed on an ongoing basis with no submission deadline. Staff states that applicants will receive follow up within 45 days after receipt of a Concept Form. Full proposals are invitation only. Invited applicants undergo due diligence and technical assistance before possible review by the Grantmaking Committee and Board of Trustees. Funding decisions are made quarterly in September, December, March, and June, and the overall application process is expected to take approximately three months.
The Health Trust does not typically fund annual appeals or membership drives, endowments, debt retirement, grants to individuals, or equipment purchases unless the equipment is part of an otherwise eligible project. There is no mandatory match or cost share stated on the current public application page.
