Broad Eligibility

Legally organized nonprofits, governments, health systems, FQHCs, RHCs, schools/education institutions, healthcare providers, community organizations, mobile-health technology providers and other capable eligible entities whose project directly serves eligible rural residents of Caroline, Culpeper, Fauquier, Fluvanna, Greene, King George, Louisa, Madison, Nelson, Orange or Rappahannock County, Virginia. Applicant headquarters need not be in the region. Adequate federal-subaward financial, reporting and clinical-credential or licensed-partner capacity required.

Atlas Analysis

Verified actual applicant-facing RHTP federal pass-through competitive subawards from Thomas Jefferson Planning District Commission and Northern Piedmont Regional PDC Collaborative, not procurement. RFA reference RFA-RHTP-2026-01, issue October 23, 2026. Approximate $11.8 million aggregate pool; no per-applicant ceiling, at least five awards, reimbursement with no advances, expenditures through September 30, 2027. No applicant match or cost share required (explicit November 3, 2026 revised FAQ); minimal direct administrative costs, no indirect-cost funding, no pre-award costs, no new construction and no reimbursement of otherwise billable services. Material published guidance discrepancy: main RFA eligibility text permits UEI by award, but the attachment checklist requests current UEI evidence and official FAQ states lead applicant must have active UEI; applicants should confirm timing in writing. Source review: updated 27-page official RFA posted October 29, 2026 (written questions now due November 13, 2026 at 5:00 p.m. Eastern), revised seven-page November 3, 2026 FAQ, and administrator announcement; reverified November 4, 2026 with November 3 revised FAQ covering no match, eligible technology models and paid purchase deposits.

Current opportunity: The Thomas Jefferson Planning District Commission (TJPDC), working with the Northern Piedmont Regional PDC Collaborative, is accepting competitive applications under Virginia Rural Health Transformation Program (RHTP) RFA-RHTP-2026-01 for mobile and hybrid care projects. The application deadline is Monday, November 23, 2026 at 5:00 p.m. Eastern Time. The RFA was issued October 23, 2026 and is an actual applicant-facing subaward competition, not government procurement.

Funding and awards

The RFA identifies approximately $11,800,000 in total program funding and anticipates at least five awards. This figure is the overall competition pool, not an individual grant amount. No individual applicant maximum is established in the RFA. The lead applicant must propose a justified first-year budget. Awards can be reduced, partially funded, or declined. Eligible project costs must be expended by September 30, 2027; the contract term begins on the executed award effective date. Awards operate on a reimbursement basis, without advances; applicants need sufficient cash flow to cover costs before reimbursement.

Who can apply and geographic restrictions

Potential lead applicants include nonprofit organizations, government entities, health systems, Federally Qualified Health Centers, Rural Health Clinics, educational institutions, healthcare providers, community-based organizations, mobile health technology organizations, and other legally organized, capable entities. The project must directly benefit eligible rural residents of one or more of these Virginia counties: Caroline, Culpeper, Fauquier, Fluvanna, Greene, King George, Louisa, Madison, Nelson, Orange, and Rappahannock. Applicant headquarters need not be in one of these counties if the project directly serves eligible rural residents there. Partners and appropriately licensed providers may collaborate on delivery. The funder expects federal-subaward financial and reporting capacity and appropriate clinical credentials or credentialed partners.

Eligible projects and significant restrictions

The RFA supports sustainable mobile care units, telehealth kiosks and access points, clinics in a box, provider-to-provider electronic consultations, rural care hubs, hub-and-spoke models, screening and diagnostic access, and supporting technology. Examples of eligible start-up costs include mobile-unit vehicles, certain equipment, limited initial personnel, data infrastructure, clinical training, technical assistance, and minor renovations if consistent with the RFA. Pre-award costs, new construction, lobbying, unrelated or duplicative costs, and replacement of funding already provided by another source are prohibited. Medicare-, Medicaid-, or other third-party-covered services may not be charged again to this program. The RFA anticipates minimal direct administrative support and no indirect-cost funding. The official revised November 3, 2026 RHTP FAQ confirms that no applicant matching contribution or cost share is required.

How to apply

Submit a complete PDF application by email to sourcing@tjpdc.org by November 23, 2026 at 5:00 p.m. Eastern Time. Include the required application cover sheet, project narrative (maximum 15 pages), implementation work plan and performance measures, detailed project budget and budget narrative, UEI documentation, most recent audit or financial statements as applicable, clinical credential documentation if applicable, and letters of commitment where a project depends materially on partners. Use RHT26-019-PDC10 in document filenames, the email subject, and correspondence. Written questions are due by November 13, 2026 at 5:00 p.m. Eastern Time. The funder anticipates award notifications January 8, 2027.

Important application clarification: The RFA eligibility narrative indicates that a UEI may be obtained by award, but the required-attachments checklist asks for current UEI documentation and the posted FAQ says the lead applicant must have an active UEI. Applicants without an active UEI should request written confirmation from TJPDC before relying on the later-award language. The revised November 3, 2026 FAQ confirms that material partners and contractors need not all hold separate active UEIs or SAM.gov registrations. It additionally confirms that for-profit applicants, technology-led nonclinical navigation models, and purpose-built mobile clinics may qualify when all RFA conditions are met; reimbursement requests may include documented manufacturer deposits after payment, subject to award contract terms and the September 30, 2027 spending deadline. Funding cannot duplicate billable medical services.
