Status, Timing, and Compensation

Eligible Professionals

Provider Requirements and Workflow
Providers perform examinations similar to new-patient or second-opinion consultations. DDB generally schedules an appointment about 14 days after contacting the provider. The examination must be performed in the provider’s office or an approved rental space, and the building and office must be ADA compliant. The provider submits a narrative report within 10 days of the examination. DDB supplies instructions, sample narratives, and access to free dictation services. The consultative examiner does not determine benefit eligibility, provide treatment recommendations, or provide follow-up care as part of the DDB examination.
How to Join the Panel
To become a consultative examination provider, complete the Wisconsin Consultative Examination form F-00154 linked from the official DDB page and submit it to the email address listed on the form. Wisconsin DHS states that DDB will then contact the provider to answer questions and begin the process of adding the provider to the statewide panel. Related Disability Case Consulting
DDB separately contracts with licensed psychologists and physicians who have training and recent experience in internal medicine, family medicine, pediatrics, neurology, psychiatry, physical medicine and rehabilitation, or occupational medicine to review disability case records. Initial consulting work is performed on DDB premises, with a hybrid schedule available after six months. Wisconsin DHS states that proposals are reviewed as needed.
