The Alaska Department of Health, Division of Public Health is entering into Alaska Breast & Cervical Screening Program Provider Agreements with qualified providers under 7 AAC 81. The program purchases covered breast and cervical cancer screening and diagnostic services for eligible enrolled women through a fee for service model.
Opportunity status and deadline: The statewide public notice was published August 5, 2026. No separate application deadline, closing time, or time zone is published. The public notice carries an archive date of June 30, 2031, which Atlas does not treat as an application deadline. A provider agreement becomes effective after both parties sign and remains in force until the provider or the State terminates it or a material agreement term changes.
Provider tracks: The State publishes a Screening Provider Agreement and a Consultant/Resource Provider Agreement. Screening services must be furnished by appropriately Alaska licensed clinical professionals with prescribing authority, including physicians, osteopathic physicians, certified physician assistants, advanced nurse practitioners, and certified nurse midwives. Consultant/resource services may include breast specialist consultation, gynecology, colposcopy, screening and diagnostic imaging, laboratory cytology/pathology, and related covered services.
Eligible provider entity types: The screening agreement identifies private for profit businesses licensed to do business in Alaska, qualifying nonprofit organizations, Alaska Native entities, and political subdivisions of the State. Providers must maintain all licenses, certifications, credentials, insurance, facility requirements, and other qualifications applicable to the services they provide. Alaska Native entities must submit the required waiver of sovereign immunity.
Required materials: The applicable signed provider agreement, Provider Profile, Privacy & Security Procedures, Federal Assurances & Certifications, applicable provider checklist, proof of Federal Tax Identification/Employer Identification Number, current Alaska business license, applicable certificates of insurance, and any entity specific required documentation. Providers must follow the current Provider Manual and approved CPT code list.
Funding and reimbursement: No aggregate per provider maximum or award range is published. Payment is fee for service for Alaska Breast & Cervical approved covered services. The approved CPT code list and reimbursement rates are revised annually based on current Medicare reimbursement rates. Alaska Breast & Cervical is payer of last resort. No mandatory match or cost share requirement is stated in the controlling agreements.
Important operating terms: Subcontracts are not allowed. Providers must comply with applicable privacy, security, reporting, record retention, quality assurance, insurance, and other state and federal requirements.
