Submission Method
The completed application and required attachments must be returned by mail to Community Foundation of Southwest Kansas, P.O. Box 1313, Dodge City, KS 67801. The Foundation expressly states that applications should not be submitted by email or fax and should be photocopy-ready without staples, paper clips, binders, or presentation folders.
Grant Conditions
Recipients must execute the Foundation’s funding agreement before disbursement, use funds only for charitable purposes, maintain reasonable expenditure records, return unused funds, acknowledge the Foundation in publicity, and submit a report no later than September 1 describing how funds were used, people served, and project outcomes when available. Grant funds may not be used for lobbying, election influence, or voter-registration activity.
