Verified September 14, 2026. K21 Health Foundation is accepting 2026 Quarter 4 grant requests for qualifying organizations whose projects benefit Kosciusko County residents and address community health and wellness.
Deadlines and application path
The required first step is K21’s five-question Pre-Grant Questionnaire in its online grant portal. The 2026 Quarter 4 Pre-Grant Questionnaire is due October 19, 2026 at 5:00 PM. After K21 reviews the questionnaire, its Grants Manager may open the full application phase or schedule a pre-application meeting. If the application phase is opened, the full application is due November 2, 2026 at 5:00 PM. K21’s current deadline page does not state a time zone, so Atlas does not infer one.
Eligibility
K21’s current eligibility page identifies three applicant pathways: nonprofit agencies with current IRS tax-exempt status and good standing with the Indiana Secretary of State; school systems serving Kosciusko County students; and governmental agencies serving Kosciusko County residents. The proposed project or program must benefit Kosciusko County residents and address health and wellness needs or opportunities.
Funding and health priorities
K21 publicly states that there is no dollar limit on the amount an organization may request, provided the applicant can clearly justify the full need. Current public guidance does not state a mandatory match requirement. Any additional portal-only requirements must be verified if K21 opens the full application phase.
K21 organizes its grantmaking around four Health Pillars: Community Culture & Wellbeing, Health Access & Opportunity, Direct Services & Prevention, and Youth Development & Activity. The strongest healthcare pathways for Atlas include projects that improve access to care, deliver direct health services, support prevention, strengthen EMS or community care, or otherwise address a documented Kosciusko County health need.
N9+ and vendor fit
Atlas Opportunity Score: 94/100. N9+ Fit Score: 88/100 conditional. The Nonagon N9+ device and platform could be a legitimate applicant-selected technology component when a qualifying organization has a documented project need involving guided remote physical examination, vitals, or telehealth-enabled clinical encounters. K21 does not specifically preapprove N9+ or any other vendor or technology, so project-specific budget treatment and allowability must be confirmed before inclusion.
K21 has documented prior equipment awards, including first-responder and AED equipment, training mannequins for emergency personnel, and healthcare training equipment that included an automated vital-sign monitor. Those awards demonstrate an equipment-funding precedent, but they do not make any specific future device or technology automatically allowable.
Submission mechanics
Applicants begin in the K21 online grant portal with the Pre-Grant Questionnaire. K21 recommends meeting with its team before submission, and Jennifer Stewart, Grants Manager, is the published contact for grant questions and community connections. The complete full-application forms and attachments are not publicly exposed before K21 opens the application phase and should not be inferred.
