Broad Eligibility

Year 2 eligibility is not yet published and will be controlled by each future Vermont RHT RFP/NOFO. Prior Year 1 opportunities involved hospitals, healthcare organizations, providers, community partners and qualified vendors depending on the solicitation.

Atlas Analysis

Atlas Opportunity Score 97/100 as a future funding watch. N9+ Future Fit 90/100 conditional because Vermont Year 1 included Telehealth and Remote Patient Monitoring solicitations; QC Healthcare Future Fit 70/100 conditional. No current Year 2 RFP/NOFO is open, so no present allowability or applicant eligibility should be inferred.

Funding Watch Status

No Year 2 RFP or NOFO is currently open. Vermont’s Agency of Human Services states that all Year 1 Rural Health Transformation Program RFP and NOFO opportunities have closed and that Year 2 opportunities will be posted on the State’s RHT opportunities page when available. Exact Year 2 posting dates are still to be determined.

Program Scale and September 2026 Implementation Update

Vermont’s Rural Health Transformation Program is a five-year federal initiative funded by the Centers for Medicare & Medicaid Services. Vermont’s Year 1 award totals $195,053,740.44, 100% federally funded by CMS/HHS. On September 10, 2026, the Vermont Department of Health announced the State’s first project from that award: $9 million to the Vermont Student Assistance Corporation for Rural Health Transformation tuition benefits supporting people training in nursing, dentistry, mental health, emergency medical services and other eligible health professions who commit to rural Vermont service.

VSAC says funding for academic year 2026-2027 is already fully allocated and that applications for the 2027-2028 academic year will begin October 1, 2026. That tuition program is an individual workforce benefit rather than a provider technology grant.

Why Year 2 Matters

Vermont’s RHT goals are to strengthen the rural health workforce, increase timely rural access to care, and use innovative strategies to improve quality while reducing costs. Year 1 solicitations included Remote Patient Monitoring, Telehealth, eConsult, Mobile Integrated Healthcare, Shared EHR, facility upgrades, care transformation, analytics and other system-transformation work. Those Year 1 solicitations are closed, but their scope shows that connected-care technology and clinical-service models are materially relevant to future Vermont RHT monitoring.

Atlas Analysis

Atlas Opportunity Score: 97/100 as a future funding watch. The program is exceptionally high-value and directly aligned with rural access, workforce and technology-enabled care, but there is no current Year 2 application or procurement to pursue.

N9+ Future Fit: 90/100 conditional. Prior Vermont RHT solicitations expressly included telehealth and remote patient monitoring, creating a strong strategic basis to watch for future connected-examination opportunities. This does not establish that the Nonagon N9+ is currently allowable or funded under any future Year 2 notice. Atlas must verify the exact next solicitation before positioning the platform.

QC Healthcare Future Fit: 70/100 conditional. Workforce, mobile integrated health, clinical transformation and access initiatives may create legitimate future service opportunities, subject to the qualifications and procurement terms of a specific Year 2 notice.
