Opportunity Overview
The Chartercare Foundation 2026 Wellness/Obesity Prevention/Nutrition Education Grant supports structured Rhode Island programs that identify, treat, and track the effectiveness of wellness programs, nutrition education, diabetes prevention, and obesity prevention services, particularly among disadvantaged populations.

Eligibility and Geography

Required materials include the proposal description, a 12-month proposal budget, the current fiscal-year organizational budget comparing actual revenue with budgeted expenses, a cover letter with an executive summary addressing the required program points, the organization’s last two audits, and the application cover sheet. The project budget must detail project income already on hand, other anticipated or applied-for funding, and expenses including personnel and percentage of time spent on the proposed program. The Foundation discourages proposals focused directly or solely on staff hires or salary and benefit budget relief.
No mandatory non-Federal match or cost share is stated. No special vendor registration or procurement requirement is stated in the public application. Applicants should confirm any technology, device, vendor, or equipment line item with the Foundation before relying on it in a budget.

Technology and Dr. Miltie Relevance
N9+ Fit Score: 30/100 conditional. N9+ is not a natural core fit for nutrition education, diabetes prevention, or obesity prevention. A limited use case could exist only where an eligible applicant builds an integrated clinical-access program in which guided remote examinations, vitals, remote assessment, or telehealth-enabled encounters materially support the continuum of care and the Foundation confirms the technology cost is allowable. Do not force N9+ into a wellness-only proposal.
QC Healthcare Fit: 38/100 conditional. QC Healthcare or other Dr. Miltie capabilities may be relevant only where an applicant-led clinical services, care coordination, or measurable access component fits the program and the associated service costs are allowable. General staffing budget relief is expressly discouraged.

This is a clear Rhode Island-only healthcare foundation opportunity with a published $25,000 maximum, broad nonprofit eligibility, a defined patient/outcomes orientation, and a practical application package. The score is moderated by the relatively small award ceiling and lack of an exact deadline time/timezone or explicit technology/equipment cost rules.
Realistic Rhode Island applicants include nonprofit clinics and community health organizations that already provide wellness, nutrition, diabetes prevention, or obesity prevention services to underserved populations. Rhode Island Free Clinic is a particularly credible candidate because its official materials describe free preventive care, wellness and education services for uninsured, low-income adults statewide; any project still must be specifically designed and applicant-controlled.
Dr. Miltie, LLC is not a direct eligible applicant under the published 501(c)(3) eligibility rule.
