Required attachments are the Board of Directors, Advisory Board and Executive Director list; the IRS 501(c)(3) designation letter; and the prior fiscal year Income and Expense Statement.

Applicants must mail one original application plus seven copies to Children’s Medical Charities Association, Attention: Grant Review Committee, 1020 S. Rockford Ave. Suite F, Tulsa, OK 74120. Only one set of attachments is required.
