Broad Eligibility

Eligible Ohio applicants include state, local and Tribal governments, institutions of higher education, nonprofit organizations, hospitals and other healthcare organizations with at least five years of experience delivering health services in one or more of the 73 counties of focus. Projects must benefit residents of those rural counties and establish or expand a clinically integrated network or Rural Health Innovation Hub.
