The County of Monmouth, through the Department of Health & Human Services Office of Monmouth ACTS, is seeking qualified organizations to provide community based service coordination and resource navigation for Monmouth ACTS ResourceConnect.
ResourceConnect is Monmouth County’s human service navigation system connecting residents with supports that may include food, housing, medical care, employment programs, mental health services, child care, and other community resources.
