The New Jersey Healthy Homes Initiative 2027 is an active New Jersey Department of Community Affairs housing production opportunity operated with the New Jersey Department of Human Services Division of Medical Assistance and Health Services. The program finances permanent, affordable, community based housing for NJ FamilyCare members experiencing or at risk of homelessness or institutionalization.
Funding and eligible applicants
The current DCA SAGE listing identifies $95,000,000 in estimated total program funding. The listing shows individual award minimum and maximum amounts as Various, so no applicant level ceiling is stated. Eligible applicants are for profit and nonprofit affordable housing developers. Healthy Homes funding is designed to be braided with approved DCA housing production funding and other suitable federal, state, county or local programs.
The controlling program guidance states that Healthy Homes applications are submitted through DCA SAGE and considered on a rolling basis. The current SAGE listing gives a submission deadline of June 30, 2027. The listing does not state a submission clock time or timezone.
Allowable uses and program structure
Healthy Homes capital funding supports construction or rehabilitation of qualifying units. The program guidance also allows fixtures, furniture and equipment for basic housing needs, such as refrigerators, stoves, beds, couches, chairs and tables. Separate repair and replacement reserves support eligible long term building and unit infrastructure costs. Capital funding is structured as a 30 year, zero interest mortgage that can be forgivable at the end of the term if the project remains compliant with the program’s Medicaid placement, rent and deed restriction requirements.
The program is intended to develop approximately 200 deed restricted rental units statewide. Units must satisfy federal Home and Community Based Services settings requirements, serve qualifying NJ FamilyCare members, and generally be integrated into the broader community with access to transportation, healthcare, behavioral health, pharmacy, education, employment and other community resources.
Application requirements
Applicants use DCA SAGE. DCA’s general SAGE instructions require current agency information, including the applicable board of directors list, SAM identifier, federal congressional district and vendor identifier, and completion of the application contacts, components, application information, application forms and budget sections. Program specific forms and expected attachments are displayed inside the SAGE application. Applicants should rely on the live SAGE record for the exact project specific attachment list before submission.
Atlas analysis
Atlas Opportunity Score: 90/100. This is a large, long horizon New Jersey housing and Medicaid aligned capital program with a clear statewide health and social determinants purpose. N9+ Fit Score: 0/100. The verified allowable uses are housing development, rehabilitation, basic housing fixtures and equipment, and related long term property costs. The controlling sources do not establish a legitimate funded role for the Nonagon N9+ device and platform or QC Healthcare. Dr. Miltie/QC Healthcare is not established as an eligible direct applicant, and no device or clinical outreach should be inferred from the program’s healthcare context.
