Urban Indian Health Institute — 2026–2027 Sweetgrass Community Grants
The Urban Indian Health Institute (UIHI) is accepting applications for its 2026–2027 Sweetgrass Community Grants, a distinct nationally focused community-health competition for eligible urban American Indian and Alaska Native organizations. Projects use Indigenous cultural, traditional, and regional approaches to education, prevention and management of chronic disease, including tobacco, diabetes and prediabetes, high blood pressure, obesity, nutrition, physical activity, and oral health.
Funding and deadline
Individual grant: $10,000. UIHI indicates that up to four Sweetgrass grants are available. The grant period is September 30, 2026 through September 29, 2027. Applications are due Monday, November 30, 2026, at 11:59 p.m. Pacific Time (PT). An applicant cash-match requirement is not specified in the controlling RFA.
Who may apply
Eligible applicants are Indian Health Service Title V Urban Indian Organizations, National Urban Indian Family Coalition members, or, on a case-by-case basis, qualified 501(c)(3) urban Indian nonprofits with majority urban Indian leadership and board whose mission includes urban Indian public health. Applicants must participate in program measurement and evaluation and a chronic disease community of practice.
Eligible activities and restrictions
Eligible initiatives include culturally adapted chronic disease curricula, traditional foods and nutrition knowledge, physical activity promotion, engagement of urban Indigenous youth and Elders, and community-led prevention planning. Reasonable personnel, project-specific salaries, travel, supplies, and services are eligible. The RFA prohibits research, clinical care, and purchases of furniture, equipment, and clinic or patient supplies. The program is distinct from the larger Sage grant opportunity.
