Montana DPHHS has opened the FY26 Community Integrated Health Pilot Site Grant under the Rural Health Transformation Program.
Applicant structure: The controlling FY26 Submittable application is open to emergency medical service agencies located in and/or serving rural areas of Montana. Eligibility is based on the agency’s ability to administratively manage a Community Integrated Health program and obtain medical director oversight. Priority consideration is given to agencies already operating CIH programs in rural or frontier areas that need funding to remain operational or expand; agencies initiating a CIH program may also be considered, and DPHHS may require proof of enrollment in a state approved CIH course before award.
Deadline: the controlling State of Montana Submission Manager page exposes a machine-readable deadline timestamp of September 15, 2026 at 23:00 UTC, which is September 15, 2026 at 5:00 PM MDT in Montana. This resolves the prior public-page clock-time ambiguity.
Allowable cost structure: Initiative 3.2 expressly permits FTE, equipment, supplies, travel and other approved expenses. Equipment, vehicles, technology and supplies purchased with grant funds must support the approved CIH scope and eligible rural or frontier service area. Equipment must be purchased through a DPHHS approved vendor or group purchasing organization and is subject to federal property management requirements, including 2 CFR 200.313.
Program fit: Montana CIH is designed to expand EMS beyond emergency transport into patient assessment, monitoring, care coordination, chronic disease support, readmission prevention and telehealth facilitation. N9+ is therefore a strong potential equipment and technology component where an applicant’s approved CIH project uses connected remote assessment, objective exam capture, monitoring or telehealth workflows and the required procurement route is satisfied.
Atlas Opportunity Score: 95/100. N9+ Direct Vendor Fit: 94/100. QC Healthcare Fit: 72/100 provisional, because clinician service allowability must be confirmed within the applicant’s approved scope and budget.
Path: Path B/C only, not TRUE Path A. An eligible EMS applicant applies and may use Dr. Miltie as a technology vendor or implementation partner where the approved project and procurement rules permit.
Current Atlas action: As of September 15, 2026, the exact deadline is verified as 5:00 PM MDT. The 48-hour cutoff for new applicant outreach has passed, so do not begin or continue outreach to previously unsent applicants. Continue permitted work only for organizations already contacted before the cutoff, and reverify applicant eligibility, medical director oversight, approved CIH scope, and the DPHHS approved vendor or group purchasing route before final application or equipment commitments.
