Broad Eligibility

Certified Rural Health Clinics physically located in fully or partially rural Michigan counties; priority to fully rural counties. Individual or multi-clinic organization-level application; each RHC only one application for this fund.

The Michigan Center for Rural Health (MCRH), with Michigan Department of Health and Human Services Rural Health Transformation Program Year 1 support, is accepting competitive applications for its Rural Health Clinic Chronic Disease and Collaborative Care Fund. Completed applications and supporting materials must be emailed to RHTP.MCRH@msu.edu before 5:00 p.m. Eastern Time on October 30, 2026. This is a financial-assistance subgrant to rural clinics, not a vendor procurement.

Individual applicant funding

A qualifying application may request up to $300,000. The same maximum applies to a combined organization-level application for multiple participating Rural Health Clinics; it is not a $300,000 award per clinic within a shared application. Awards are competitive and subject to final state approval. No matching contribution is required. Funding is reimbursed for approved eligible expenses after expenditure and documentation, not paid in advance. Grant expenses may start only following an executed agreement and approved workplan and budget, and must be expended by September 30, 2027.

Eligibility

Applicants must be Certified Rural Health Clinics in Michigan counties designated fully or partially rural. Clinics in fully rural counties receive priority. A qualifying health system may apply for multiple eligible sites in one application with a combined $300,000 cap, or its eligible clinics may submit individual applications. Every participating clinic must be identified by name, location, and CMS Certification Number; each clinic may appear on only one application for this particular fund. Other RHTP programs may be pursued separately without duplicating charges.

Eligible work and restrictions

The fund supports additional or expanded chronic disease prevention and management, cross-sector partnerships, Community Health Worker integration, patient screening and follow-up, evidence-based prevention, care coordination, performance tracking, clinical and community referrals, project-related training, and approved supporting technology. All funded clinics must address required priority areas and plan for sustainability. Expenses must be in the approved project budget, reasonable, and newly additive; routine costs replacing existing support, duplicate charges, pre-agreement spending, unrelated expenditures, real estate or vehicle purchases, construction or major renovations, billable patient-care costs, and prohibited lobbying or political activities are excluded. Technology, indirect or administrative costs, and some equipment require additional review.

How to apply

Download and complete the official fillable PDF application and review both the Application Guide/FAQ and Eligible and Ineligible Uses of Funds Guide. The form requests clinic identification and CCNs; project contact; community need and populations served; proposed activities; community partnerships; CHW integration; screening and care coordination; sustainability; staffing readiness; anticipated outcomes; a requested amount and high-level budget; any overlapping RHTP support; and an authorized certification. Detailed workplan and final project budget are developed with MCRH if selected. Save and reopen the PDF to check entered answers, then email it with supporting materials to RHTP.MCRH@msu.edu before 5:00 p.m. Eastern Time on October 30, 2026.
