The one-page Community Impact Grant application requires the applicant’s EIN, legal organization name, year founded, current operating budget, executive director and contact information, principal address, project name and purpose, amount requested, total project cost, geographic area served, and signatures from both the board chairperson and executive director. The same program or project may not receive another Community Impact Grant within 12 months of the first award.
Match, Cost Share, and Funding Rules
