Required next actionVerification hold for concept-paper/package completeness, not for the unpublished time zone. BCBSM Foundation's current official Community Health Matching page verifies the Winter 2027 concept paper deadline of October 28, 2026 at 11:59 p.m., the January 6, 2027 at 11:59 p.m. full-application cutoff, Michigan nonprofit eligibility, mandatory external match tiers, proposal components, and current cost exclusions. The page does not publish a time zone, so do not infer one. Keep unpublished until the live concept-paper intake form and any applicant-only certifications/questions can be reviewed. Because a mandatory external match applies, do not advance a submission without verified match compliance and Marc approval where applicable.

Broad Eligibility

Michigan-based nonprofit community organizations. Mandatory external match varies from 10% to 60% based on organization budget and Medicaid/uninsured patient share; applicant cannot self-match.

Atlas Analysis

Formal Atlas Opportunity and N9+ scoring remain deferred because the live concept-paper intake form and any applicant-only certifications/questions have not been independently reviewed. The missing published time zone is recorded as unknown but is not, by itself, the hold reason. Current official materials otherwise verify the Winter 2027 dates and 11:59 p.m. cutoff, Michigan nonprofit eligibility, mandatory external match tiers, proposal package requirements, and cost exclusions. Do not advance a submission without verified match compliance and Marc approval where applicable.

Opportunity Overview

The Blue Cross Blue Shield of Michigan Foundation Community Health Matching Grant Program supports Michigan nonprofit community organizations working with health professionals and community leaders to develop population-led, community-inspired programs or implement evidence-based programs addressing community-identified needs with measurable health outcomes.

Funding and Deadline

The program offers up to $25,000 per year for two years; typical awards are $15,000 per year for two years. For the Winter 2027 schedule, the concept paper is due October 28, 2026 at 11:59 p.m. and full applications are accepted until January 6, 2027 at 11:59 p.m.. The controlling public page states that all deadlines end at 11:59 p.m. but does not publish a time zone, so Atlas does not infer one.

Eligibility and Match

Applicants must be nonprofit community organizations based in Michigan. A mandatory match applies and the applicant cannot serve as its own match. For organizations with budgets under $10 million, the required match is 25% when less than 30% of the patient population is Medicaid/uninsured and 10% when more than 30% is Medicaid/uninsured. For organizations with budgets over $10 million, the required match is 60% when less than 30% of the patient population is Medicaid/uninsured and 25% when more than 30% is Medicaid/uninsured.

Application and Cost Rules

The applicant first requests a concept paper through the Foundation’s published concept-paper form. If invited forward, the proposal package includes the project narrative, participant enrollment table, detailed budget and justification, co-funder letters, project timeline, resumes, organizational mission/history/services, IRS 501(c)(3) determination letter, and the most recent tax return or audited financial statement. The Foundation does not support computer equipment including hardware and software, one-time consumables such as medical and office supplies, furnishings, or capital expenses. Indirect costs are not paid except for limited fringe benefits; tuition and continuing education units are excluded, and conference travel is capped at $1,000.

Atlas Strategic Analysis

Formal Atlas Opportunity Score: deferred while the live concept-paper intake form and any applicant-only certifications/questions remain unverified.

N9+ Fit Score: deferred. The program has a strong healthcare-access focus, but the current budget rules expressly exclude computer hardware/software and capital costs. Atlas therefore does not position N9+ acquisition as a funded cost. A narrow programmatic role could be evaluated only if an applicant already has allowable resources and the Foundation confirms the proposed structure. Dr. Miltie, LLC is not an eligible prime applicant. Any QC Healthcare or other contracted-service role would require applicant-specific allowability and budget confirmation.

Verification Notes

No solicitation number or published deadline time zone was identified on the controlling page. Applicant-specific budget size, Medicaid/uninsured percentage, match source, co-funders, project design, personnel, tax/governance documents, and financial records must be provided and verified by the applicant. Because a mandatory non-Federal match applies, Atlas will not treat Dr. Miltie as a direct applicant or advance any Dr. Miltie direct-submission path without Marc approval or a verified applicable waiver.
