The Community Foundation of Greater Fort Wayne has published the 2026 Rising Together Healthy Mind & Body grant cycle for nonprofit organizations whose missions align with healthcare, mental health, food and fitness, basic needs, and related work that helps Allen County residents achieve better mental and physical health.
Application timing and funding
The Healthy Mind & Body application window opens October 1, 2026 and closes November 1, 2026 at 11:59 PM EST. Rising Together requests above $50,000 will not be considered. The Foundation states that the average Rising Together grant is approximately $20,000, and first-time Rising Together grantees are typically awarded $5,000.
Eligibility
Applicants must be 501(c)(3) nonprofit organizations, must have existed for at least three years, and must either serve at least 50% of their people in Allen County or provide a unique service to the county. Applicants may not require participation in religious programming for people to receive services and may not require employees to sign a statement of faith. An organization may receive only one Rising Together Grant per calendar year.
Funding use
Rising Together Grants are unrestricted operating-support grants. The Foundation states that these grants may fund the organization in any way it chooses, subject to the published program and IRS restrictions. The program does not fund medical or academic research. Grant funds may not be used for lobbying, prohibited partisan political activity, impermissible individual travel/study grants or subgrants, or non-charitable/non-educational purposes.
Application requirements
Applicants must first complete the Rising Together Eligibility Quiz. The application requests strategic alignment with the Healthy Mind & Body pillar, operational-support amount and prior Rising Together funding, governance and board information, organizational staffing and professional-development information, service-population demographics, collaboration and impact information, and extensive financial information. Required materials include an organizational chart, current and prior fiscal-year operating-budget information, the previous two years of balance sheets, and income statements for the current and two previous fiscal years. A budget is required. A fund-development plan may be uploaded optionally. The Foundation states that an audit is not required.
Evaluation priorities
The published rubric assigns 30 points to governance, 20 points to people and HR, 20 points to programs, and 30 points to finance and fundraising. Strong applications should therefore document board oversight and accountability, staff development and stability, measurable impact and collaboration, and sound finances with revenue diversity, liquidity, and reserves. Atlas package work should be built around those scoring factors rather than relying only on the health-program narrative.
Match and submission
No mandatory match or cost-share requirement is stated in the current Rising Together guidelines. Applications are submitted through the Community Foundation’s online grant portal. The Foundation may adjust the requested amount during review.

The Healthy Mind & Body pillar is directly aligned with healthcare and mental-health organizations, and the unrestricted operating-support structure gives eligible grantees broad discretion over lawful organizational spending. A Nonagon N9+ remote physical examination platform could be a defensible applicant-selected use where an eligible nonprofit has a real clinical-access, remote-exam, vitals, or telehealth-enabled care need, but the Foundation does not specifically name N9+ or medical technology as a preferred use. Ordinary initial outreach must therefore focus on the verified funding opportunity and Atlas grant assistance, not on selling N9+.
