The grant may cover up to 50% of eligible hydro-vacuum excavation equipment costs. The official Form 26-026 requires municipality and authorized-contact information, applicant class, surrounding communities the applicant will assist, a cost estimate, the number of unknown service lines to be reclassified, Technical/Managerial/Financial Capability Survey status, and authorized local-official certification. A cost estimate must be submitted. Costs incurred before a signed DWEE agreement are ineligible, awardees must assist other communities with unknown-line reclassification, and federal Uniform Guidance conditions apply. Submission is by mail or email to DWEE. No N9+, QC Healthcare, or direct Dr. Miltie pathway is established.
