Health Foundation of South Florida Rolling Access to Care Grants provides a year round funding pathway for eligible organizations serving South Florida. The Foundation welcomes ideas aligned with its mission and uses an initial Request Inquiry Form in the GOapply grant portal to determine whether a concept should advance into collaborative proposal development.
Eligibility and geography
Current Foundation guidance identifies eligible applicants as 501(c)(3) nonprofit organizations and local or state governmental agencies. The proposed project must exclusively serve residents of Broward, Miami-Dade and/or Monroe Counties.
Access to Care priorities
The Foundation’s Access to Care strategy seeks to increase routine and preventive care for underserved communities. Current areas of interest expressly include expanding alternative delivery models and safety net clinics, using digital health technology to increase access, patient engagement and health outcomes, supporting alternate access points such as nurse managed health centers and telehealth, and expanding the capacity and infrastructure of safety net clinics.
Funding, match and timing
Concept proposals are accepted on a rolling, year round basis, subject to limited exceptions. The Foundation does not publish a fixed rolling deadline, deadline time, or time zone. Current public guidance also does not publish a universal applicant award ceiling; funding requests vary with project size and scope. No mandatory match or cost share requirement is stated in the current public rolling guidance. Applicants should confirm any project specific conditions during the Foundation’s co-development process.
Submission process and possible documents
The first step is a Request Inquiry Form through the Foundation’s GOapply grant portal. If a request appears aligned, Foundation staff may invite the applicant into co-development and set a deadline for the final proposal. Depending on the request, final proposal materials may include a proposal narrative, budget and budget narrative, work plan, Board of Directors contact list, IRS 501(c)(3) determination letter, Form 990, and financial statement. The current public guidance does not expressly state medical device, technology vendor, or third party vendor cost rules, so applicants should confirm those costs before committing them to a budget.
Atlas fit analysis
Atlas Opportunity Score: 95/100. N9+ Fit Score: 95/100, conditional. This is a strong technology enabled access opportunity because the Foundation explicitly identifies digital health technology, telehealth, alternate access points, and safety net clinic capacity as areas of interest. A legitimate role for the Nonagon N9+ device and platform may exist within an applicant led digital health or telehealth access project, subject to Foundation approval and confirmation that the proposed technology and vendor costs are allowable. Dr. Miltie is not treated as the direct applicant.
