The current application page requires the most recent three years of IRS Form 990 filings, IRS Form 1023 with attachments, the IRS 501(c)(3) determination letter, the most recent independent audit filed with the Connecticut Department of Consumer Protection Public Charities Unit, distribution guidelines with an anticipated budget, and internal controls governing qualifying use of funds.
