The current application materials call for a completed application, a list of officers and board members and website if applicable, a detailed project budget, an IRS 501(c)(3) or 509(a) determination letter when applicable, and the organization’s most recent year end financial statement. The application also asks for the project need, mission alignment, outcomes, strategies, resources and other funding, partnerships, evaluation, and sustainability for a continuing project.
