Opportunity Overview
The Arizona Department of Health Services State Loan Repayment Program October 2026 Supplemental and Renewal Cycle is a state-administered healthcare workforce opportunity for a narrow group of qualifying primary care providers. This is not a broad new-applicant cycle. The current Arizona Administrative Code allows qualifying renewal or return-to-participation applicants to apply by October 1 each year and allows a provider who submitted an initial application in the same calendar year but was not approved in the initial allocation process to submit a supplemental initial application for the October allocation.
Funding and Deadline
The deadline is October 1, 2026. The current rule publishes the date but does not state a clock time or time zone, so Atlas does not infer one. Under R9-15-208, October allocations occur only if additional loan-repayment funds remain after the earlier allocation processes.
Funding varies by provider type, contract year, full-time or half-time service, and HPSA score. For physicians and dentists, the maximum for an initial two-year full-time period is $65,000 at a HPSA score of 18 to 26, $58,500 at 14 to 17, and $52,000 at 0 to 13. For pharmacists, advanced practice providers, and behavioral health providers, the corresponding initial two-year full-time maximums are $50,000, $45,000, and $40,000. Renewal amounts are lower by contract year, and ADHS may offer a lesser amount if funds are insufficient.
October Eligibility
October eligibility is applicant-specific. Current rules allow qualifying categories that include providers completing the initial two-year service period under specified HPSA and continuation conditions; current participants completing three or more years and continuing; prior participants who completed at least two years and are resuming; providers previously denied continuation because funds were unavailable; and same-calendar-year initial applicants whose compliant application was denied because funds were unavailable or specified employer or service-site participation caps were reached.
The current ADHS Provider Management System lists eligible SLRP provider types including physicians in designated primary care specialties, qualifying nurse practitioners and physician assistants, certified nurse midwives, pharmacists, general dentists, and specified behavioral health professionals. Applicant and service-site requirements in the controlling rules still apply.
Match, Service Obligations, and Telemedicine
Atlas did not identify an applicant cash match or cost-share requirement in the controlling rules. Participation instead carries qualifying-service, service-site, verification, and program-compliance obligations. The rules also contain supplemental verification requirements for primary care services delivered by telemedicine, including HPSA or Arizona Medically Underserved Area conditions for originating and distant sites as applicable.
Submission
Applications are administered through the ADHS Primary Care Office Provider Management System. The current portal provides State Loan Repayment Program provider registration and separate site-administrator functions for employment verification and service and encounter verification. The rules state that renewal and supplemental initial applications are accepted no more than 30 calendar days before the applicable submission date.
Atlas Analysis
Atlas Opportunity Score: 88/100. This is a high-value Arizona healthcare workforce opportunity with particular relevance to rural, underserved, primary care, dental, pharmacy, and behavioral health recruitment and retention.
N9+ Fit: 0/100. QC Healthcare Fit: 0/100. Dr. Miltie direct-vendor/applicant fit: 0/100. The program repays qualifying educational loans in exchange for service obligations. Although the rules address telemedicine service verification, they do not establish funding for the Nonagon N9+ device and platform, QC Healthcare, or a Dr. Miltie vendor role, so Atlas does not force a technology fit.
Next Action
Potential applicants should confirm that they fit one of the October renewal, return-to-participation, or same-calendar-year supplemental-initial categories and should verify current service-site, HPSA, loan, and employment documentation in the ADHS portal before submission. Brand-new applicants who did not apply in the earlier 2026 initial cycle should not treat October 1 as a general initial-application deadline.
