Open now. CommonSpirit Health is accepting applications for its 2027 Community Health Improvement Grants through September 18, 2026. The program funds defined projects led by qualifying 501(c)(3) nonprofit organizations, or applicants using an eligible 501(c)(3) fiscal agent, to improve community health, health equity, and local service systems for underserved or vulnerable populations.
Arizona Participating Hospitals and Award Ranges
Arizona participating hospitals include three granting clusters. Chandler Regional Medical Center, Mercy Gilbert Medical Center, and Arizona General Hospital – Mesa offer awards of $40,000-$150,000. St. Joseph’s Hospital & Medical Center and Arizona General Hospital – Laveen offer awards of $30,000-$150,000. Yavapai Regional Medical Center in Prescott offers awards of $20,000-$50,000.
Arizona Funding Priorities
Priority needs vary by hospital cluster. Arizona priorities include access to care; mental and behavioral health; substance use; chronic conditions including cardiovascular disease, diabetes, obesity and chronic kidney disease; housing and homelessness; food access; dental health; cancer; maternal and preterm-birth outcomes; violence and injury; heat; and related community-health needs identified in each hospital’s current CHNA.
Eligibility and Project Rules
Applicants must be IRS 501(c)(3) nonprofit organizations in good standing or use a qualified 501(c)(3) fiscal agent. Projects must address one or more significant needs in the participating hospital’s CHNA or implementation strategy, serve communities within the hospital service area, and sustain project activity for 9-12 months. Requests of $20,000 or more require two or more substantive collaborating partner organizations in addition to the applicant.
Proposal budgets may include equipment purchases up to $5,000 plus computer equipment up to $2,500, with total equipment costs not exceeding 50% of the request. Indirect costs up to 10% are allowable. The program does not fund capital projects, unrestricted operating support, lobbying or political campaigns, or contracts for services.
