Payment-Model Participation Opportunity — Not a Grant
The Centers for Medicare & Medicaid Services Innovation Center (CMMI) accepted state Medicaid program applications for the GENErating cost Reductions fOr U.S. Medicaid (GENEROUS) Model through September 10, 2026. GENEROUS is a voluntary Medicaid prescription-drug pricing model under Section 1115A of the Social Security Act. It is not a grant program and does not provide an award for equipment, telehealth deployment, clinical services, or vendor purchases.
Application Window Closed
The CMS state application deadline was September 10, 2026 and has now passed. CMS’s April 29, 2026 deadline-extension notice states that states that applied must finalize participation agreements by September 30, 2026. The public extension notice states the application date but does not state a replacement clock time, so Atlas does not infer one.
Who Can Participate
CMS states that all states and U.S. territories participating in the Medicaid Drug Rebate Program may apply, subject to CMS review and the legal and operational requirements of the model. Participating states must secure CMS authorization to enter necessary supplemental rebate agreements, including the applicable State Plan Amendment process, and implement operational, systems, managed-care alignment, coverage-policy, reporting, and model-compliance requirements.
What the Model Does
GENEROUS is designed to lower state and federal Medicaid prescription-drug spending through CMS-led negotiations with participating manufacturers. Participating manufacturers provide supplemental rebates intended to align net Medicaid prices for covered outpatient drugs with selected international prices, while participating states adopt negotiated coverage criteria and related model terms.
N9+ and Dr. Miltie Fit
N9+ Fit Score: 4/100. GENEROUS is a drug-pricing and supplemental-rebate model. Health care providers are not model participants, and the opportunity does not establish a funding mechanism for N9+ devices, remote physical examinations, telehealth, RPM, or QC Healthcare clinical services. Atlas should not force N9+ into this opportunity.
Dr. Miltie direct status: Not eligible as the state applicant. The state-participation pathway is limited to eligible state and territorial Medicaid programs. No direct Dr. Miltie application, bid, or vendor submission is identified in the state participation opportunity.
Atlas Analysis
Atlas Opportunity Score: 18/100. This is a legitimate CMS Innovation Center participation model, and the state application window closed September 10, 2026, but its applicant universe is limited to state Medicaid programs and it has little direct N9+/QC Healthcare commercial fit. Publication is primarily for federal funding-and-model intelligence completeness. No new applicant outreach or applicant-entry work should be initiated because the state application window is closed.
