Maine Rural Health Transformation Program — Community Health Worker Expansion

The University of New England (UNE), acting on behalf of Maine DHHS, released the Rural Health Transformation Program (RHTP) Community Health Worker (CHW) Initiative Request for Applications on November 2, 2026. The funding expands, integrates, trains, deploys and sustains community health workers serving rural Maine residents through health and social service delivery partnerships. This is an applicant-facing funding competition, not a government equipment or consulting procurement.

Applicant awards and period

The controlling RFA anticipates $110,000 per award for the initial nine-month period, January 1 through September 30, 2027. This is an anticipated individual award amount, not a universal maximum or guarantee. Continuation contracts may be renewed annually through September 30, 2031 subject to CMS funding. Maine’s approximately $190 million overall federal RHTP allocation is NOT an applicant-level award.

Who may apply

Eligible proposal types include community-based organizations, healthcare providers and other qualified organizations able to deploy CHWs to serve rural populations. The program anticipates at least three awards in each Maine geographic public health district, including at least one community-based organization and one healthcare provider where feasible. Applicants in partially rural Androscoggin, Cumberland, Penobscot and York counties must explicitly demonstrate service to rural patients. Applications covering more than one public health district require separate submissions for each district. Expansion in Maine’s five Tribal communities is being handled outside this RFA process.

Restrictions and program requirements

Funds cannot supplant existing state, federal, Tribal, local or private support for personnel, services or infrastructure. Awardees must ensure CHWs complete recognized core competency training or certification. Certain CHW and supervisor training and certification costs are separately covered by Maine RHTP and should not be included in the proposed budget. Use the UNE Excel budget template and follow the template instructions for eligible costs. The RFA does not state a standard cash-match percentage.

Application and deadline

Applications are due November 30, 2026, at 5:00 p.m. Eastern Time. Questions to chwrfa@une.edu are due November 9 at 5:00 p.m. ET, with answers scheduled by November 16. The RFA asks for organizational and district information; a rural population-needs statement of up to 400 words; the top three CHW roles and activities (up to 500 words); CHW team integration, supervision and partner contacts; training needs (up to 250 words); impact measures (up to 250 words); sustainability (up to 250 words); an Excel budget; and an authorized applicant confirmation. The detailed review framework totals 100 points.

How to apply

Review the complete seven-page RFA and download the official Excel budget template from UNE. Submit the online application using the UNE REDCap form. Do not upload zipped files. Check the current form, template budget line items and any posted November 16 clarification responses before submitting.
