Nevada Rural Reentry RHIT: Rural Health Transformation Program Funding

Nevada Medicaid, through the Nevada Health Authority, released its Nevada Rural Reentry Rural Health Innovation and Technology (RHIT) Request for Applications on October 29, 2026. This is a competitive applicant-facing RHTP financial-assistance opportunity for digital and clinical workflow readiness to serve rural adult or youth correctional reentry populations. It is a distinct competition from the Nevada Rural Reentry FLEX program.

Applicant award: no fixed individual subgrant maximum or minimum is published in the controlling RFA. The $5,000,000 displayed on the application is the entire available program pool, not a per-applicant grant. Awards are subject to proposal review, approved scope, available funding and CMS conditions.

Eligible applicants: eligible Nevada rural county governments, rural adult/youth correctional or detention facilities, and qualified organizations partnering with those facilities. County governments should apply on behalf of sheriff, jail or juvenile services departments. Non-rural leads must demonstrate direct benefits for eligible rural Nevada populations and obtain partner letters when required.

Funding uses: approved jail-management and health-record information readiness, interoperability, cybersecurity, connectivity, secure care coordination, Medicaid enrollment and prerelease workflow planning, staff training and EHR migration preparation. Full statewide EHR acquisition and implementation are reserved for a possible later budget period, subject to approval; BP1 funding does not automatically authorize those purchases.

Payment and restrictions: funding is reimbursement-based. No purchases, vendor agreements or charges before execution of a subaward and budget approval. Indirect costs are prohibited; approved administrative costs may not exceed 10% of a subaward. Do not request costs already supported by FLEX or another source; any separate FLEX application must have its own budget and activities. Complete approved work and expenditures by August 30, 2027; additional budget periods are not guaranteed.

Applications due: December 15, 2026. The RFA does not specify a precise closing hour or time zone. Submit the signed cover sheet, work plan, itemized RHIT budget and narratives, performance measures and reporting plan, quotes/cost evidence, applicable partner letters, technical documentation and certifications to MedicaidReentry@nvha.nv.gov. Use subject Reentry RHIT RFA – [County/Organization] – [Project Title]. Questions are due November 23, with answers scheduled November 30. The full application and attachment links are available on the official Nevada Medicaid Reentry portal.
