Rhode Island RHTP Alternative Payment Model Incentive Payments – Program Year 1
The Rhode Island Office of the Health Insurance Commissioner (OHIC) is accepting applications for time-limited financial incentive payments under the Rural Health Transformation Program (RHTP). This is an applicant-facing provider incentive program, not a traditional competitive project grant and not a government procurement contract. Approval and payment are conditional on provider eligibility, alternative payment model (APM) commitments, and quality/cost reporting.
Application deadline and expected payments
Application deadline conflict (verified October 23, 2026): the live OHIC-linked application states November 2, 2026, at 5:00 p.m. Eastern, while OHIC’s FAQ updated October 22 states December 2, 2026, at 5:00 p.m. Eastern. Applicants should confirm the controlling deadline directly with OHIC and should not rely on the later date until the discrepancy is formally resolved. For Program Year 1, OHIC estimates $25,000-$50,000 per eligible primary care practice site annually and hospital payment estimates that conflict within OHIC’s October 22 FAQ: $90,000-$150,000 in one answer versus $250,000-$500,000 in another. OHIC clarification is required before treating either hospital range as authoritative. These are preliminary and conflicting estimates, not fixed minimums or guaranteed individual maximum awards. Final amounts depend on the number of qualified providers, available funding pools, and OHIC review. Payments may be monthly or quarterly. The $156,169,931.19 statewide CMS allocation is not an individual provider award.
Who qualifies
Eligible participants are Rhode Island hospitals and primary care practice sites meeting OHIC’s rural or rural-serving test and the program’s APM and quality-reporting conditions. A provider may qualify based on federal rural-facility status, physical location in designated rural areas, at least 18% of patients from listed rural ZIP codes, certain state service responsibilities, or sole-source rural services. The OHIC FAQ states that all Rhode Island hospitals and FQHCs qualify as rural health providers. Health systems, parent organizations, IPAs, ACOs, or other authorized entities may submit consolidated applications on participating providers’ behalf.
Core commitments and spending restrictions
Primary care sites must participate in, or commit to future participation in, a qualifying primary care capitation or total-cost-of-care arrangement and report prescribed quality and APM participation information. Hospitals may qualify through specified downside-risk ACO/AE participation or good-faith readiness work toward a hospital global budget or qualifying risk arrangement, with aligned quality reporting. Hospitals taking a readiness pathway must explain intended incentive uses. Primary care practices may use payments toward staffing, infrastructure, and operational changes needed to implement advanced APMs. Providers may lose unpaid installments or face recoupment for materially inaccurate information, noncompliance, or use inconsistent with certifications. Cost share, complete eligibility attestations, and all workbook submission conditions need final confirmation from the live application.
