Opportunity Overview
Vitalyst Health Foundation is accepting 2026 Systems Change Grant applications from Arizona collaboratives that are ready to implement a defined systems-change strategy. Eligible-stage collaboratives may have completed planning through a prior Spark Grant or may otherwise be prepared to move directly into implementation.
Funding and Deadline
Vitalyst offers up to $180,000 total per Systems Change Grant, with four grants available and a grant duration of three years. Applications are due October 29, 2026 at 5:00 PM MST-AZ (Arizona Time). Finalist presentations are scheduled for January 11-15, 2027, and award notification is scheduled for March 26, 2027.
Evaluation and Partnership Structure
Vitalyst evaluates the systems-change project design, collaborative composition and partner roles, representation of the impacted community, health outcomes and disparities, evaluation metrics, and the budget narrative. Letters of Commitment are part of the collaborative criteria. Vitalyst strongly encourages prospective partners to explore alignment through its Discovery process before applying.

Atlas Opportunity Score: 78/100. This is a substantial Arizona community-health systems-change opportunity, but it is not a direct clinical-service or capital-equipment funding lane. Vitalyst states that its partnership grants do not fund programmatic or direct service, capital expenditures or long-term capital assets.
