Required materials and process: Current guidance identifies school/district approvals, a GO Smart organization profile, IRS tax-exempt determination documentation, FEIN, UEI, Kentucky Vendor Number, and a DocuSign signature page involving school and district administrators. Submitting the signature page does not itself submit the grant application; the applicant must return to the online application and click Submit.
