Applications are submitted through the Black Hawk County online Form Center. The application requires organizational and contact information, an IRS tax-exempt determination letter, mission and service information, the amount requested, project name and description, recent financial statements, and a detailed project budget. If audited financial statements are unavailable, additional business-plan and board or officer information is required. Applicants must also address community need, expected impact, sustainability, leveraged or matching funds, and other Black Hawk County funding received since July 1, 2025.

Applicants selected for consideration should be prepared for a ten-minute presentation to the Board of Supervisors. Evaluation factors include demonstrated need, goals, impact, number of residents served, partnerships, sustainability, return on investment, budget, cost per resident served and multiple funding sources.

Post-Award Requirements

Funded organizations enter into a contract with Black Hawk County. Services must be performed and required documentation provided before payment. Annual reporting and periodic updates may be required, and failure to provide required reporting can affect future funding eligibility.
