The grant supports establishment of a CDC-recognized National DPP delivery organization, referral pathways with health systems and clinics, community promotion and recruitment, a discovery session, launch of a first National DPP cohort, data collection and reporting, and collaboration with DHS on evaluation and performance measurement.

DHS identifies staff time, project-related meeting expenses, public health evaluation, office and administrative supplies, and consultant or contract services needed to implement the project as common allowable costs. Clinical care, treatment, medications, research, construction, and capital equipment costing more than $5,000 are expressly unallowable.
