Submission: Applicants must complete Sunshine Health’s official five-page grant application and email the completed application to SM_FL_CommunityConnections@SunshineHealth.com. Sunshine Health states that incomplete, inaccurate, handwritten, or scanned applications will not be accepted.
Eligibility
Applicants must have active 501(c)(3) status in Florida and must not have received funding from Sunshine Health or Centene Corporation since September 1, 2024. Sunshine Health states that prior in-kind donations do not disqualify an organization.
Program focus
The grant supports Florida nonprofit programs addressing health-related social needs and social determinants of health. Sunshine Health’s published examples include food, housing, transportation, workforce training, and other community needs that affect health and well-being.
The application asks organizations to describe their mission, program, community need and primary goal, whether the program is new or existing, how Sunshine Health Medicaid members will be identified or tracked, reporting methods and capacity, and examples of expected impact reporting.
Required application information
The official application requests the organization’s legal name and DBA, EIN, mission, primary contact, organizational and service-area information, primary and additional counties served, years of operation, approximate people served per month, prior Sunshine Health or Centene funding, program description, statement of need and goal, program status, client-tracking approach, reporting capacity, expected impact reporting, and authorized representative certification and signature.
Post-award reporting
After grant funds are used, funded organizations must submit an impact summary addressing clients served, measurable outcomes, and photos. Sunshine Health states that no protected health information or personally identifiable information will be exchanged.
