Broad Eligibility

Eligible lead applicants include New Hampshire FQHCs and FQHC Look-Alikes, CMS-certified Rural Health Clinics, rural physician- or nurse-practitioner-led comprehensive primary care practices, and Critical Access Hospitals operating rural primary care/RHC services or participating through an eligible primary-care partnership. Partners may include oral health, EMS, behavioral health, home health, community-based organizations, and other entities. Rural eligibility must satisfy the RFA's HRSA/designation/beneficiary criteria.
