Obici Healthcare Foundation is accepting Cycle 3 proposals for its 2026–2027 Healthy Behaviors Partnership Development, Planning, and Implementation Grants. The program supports community-driven, multi-sector approaches to increase access to healthy food and opportunities for physical activity. The Foundation’s service area expressly includes Gates County, North Carolina.
Funding tracks
Partnership Development Grants: up to $5,000 for 6 to 12 months to help a qualified applicant convene multi-sector partners and build collaborative capacity.
Planning Grants: up to $25,000 for up to one year for planning activities and related expenses such as meetings, consultants, assessment and data collection, resident engagement, and pilot activities.
Implementation Grants: up to $100,000 per year for up to three years to implement programs, promotional strategies, educational campaigns, policies, systems, and environmental-change strategies. Second and third years of funding are contingent on annual progress.
Deadline: December 1, 2026 at 2:00 PM ET. Funding Cycle 3 is subject to availability of funds.
Eligibility and collaboration requirements
Applicants must be 501(c)(3) nonprofit organizations or government agencies located in and serving communities or populations within the Foundation’s service area. Each application must be submitted by one eligible organization serving as the collaborative convener and fiscal intermediary. The applicant generally provides a designated project coordinator or director and must have at least five committed partners representing multiple sectors such as health, government, education, business, and nonprofit organizations.
The Foundation encourages meaningful participation by residents most affected by lack of access to healthy food, limited physical-activity opportunities, and health disparities. Implementation applicants are required to engage affected residents and should demonstrate a ready-to-implement initiative with significant and sustainable community or regional impact.
Allowable costs and match
Funding may support staff time, assessment activities, meeting expenses, educational activities, advocacy, project-related supplies and equipment, communications, and consulting support. Resource sharing with significant partners may include mini-grants, consulting payments, and subcontracts.
Matching funds and in-kind support are encouraged but not required.
Funds may not support lobbying or political programs, biomedical, clinical or educational research, scholarships, endowments, supplanting existing support, individual assistance, annual fund drives, projects outside the Foundation service area, or direct medical or social services already funded through third-party reimbursement.
Submission requirements
Applications are accepted only through the Foundation’s online application process. All tracks require the official Proposal Cover Sheet, a community-context section, a description of the proposed collaborative, the applicant’s 501(c)(3) or government exemption determination letter, and the organization’s most current Form 990, annual audit, or financial report. Track-specific narratives, work plans, budgets, and supporting materials also apply.
Implementation applicants must additionally provide a readiness statement, project description, detailed first-year and high-level later-year work plan, line-item budget and narrative, letters of support from four core partners combined into one document, and an applicant board-member list. Finalists may be asked for MOUs confirming partner commitments before funding is finalized.
Applicants are strongly encouraged to engage Foundation staff for pre-submission consultation and technical assistance at least two weeks before the proposal deadline.
